Payment. Comprehensive new patient initial intake includes (minimally):           
1.  1 to 1.5 hour Initial Visit
       a. Includes a detailed Medical history Intake and out-of-office history review
       b. Recent Laboratory work will be reviewed and interpreted
       c. More labs may be recommended based on the patient's condition

2.    Dr. Moody will review all your medications and supplements.
       a. She will review potential side effects.
       b. She will make sure you are taking the right form and sufficient quantities of necessary
            vitamins and minerals
       c. She will also review if there are any drug/nutrient/herb interactions.  
       d. She will review if any medications being taken cause specific nutrient depletion.

3.    Treatments plan (this is a very individualized option and it cannot be said exactly what the
        treatment will consist of without the proper intake.)

4.    This fee also includes the first 30 minute follow-up visit.

Fountain of Health Contemporary Medicine, PLLC is a cash-pay office, and therefore, payment is expected at time of service. We require payment in full at the time services are rendered.
